Occurence Categorization

1.1.1 Category 1 Occurrence (highest level)

1.1.2 Accident involving loss of aircraft and/or death to a person associated with the task Requires:

  • immediate notification to management
  • immediate institution of investigation procedures
  • immediate implementation of accident or emergency response plan
  • immediate quarantining of relevant documents
  • the obtaining of witness and participant contacts/statements as soon as possible
  • immediate notification to relevant external agency (ATSB, NTSB, FAA, CASA, PHMSA, occupational health authority or any other identified authority). (see relevant regulation)
  • recording in IRIS

 

1.1.3 Category 2 Occurrence

1.1.4 Accident involving dangerous goods, damage to aircraft and/or equipment and/or serious injury to a person associated with the task Requires:

  • immediate notification to management
  • immediate institution of investigation procedures
  • the obtaining of witness and participant contacts/statements as soon as possible
  • immediate notification to relevant external agency (ATSB, NTSB, FAA, CASA, PHMSA, occupational health authority or any other identified authority). (see relevant regulation)
  • recording in IRIS

 

May require (depending on severity of accident)

  • implementation of accident or emergency response plan
  • quarantining of relevant documents

 

1.1.5 Category 3 Occurrence

Incident involving, or potentially involving, dangerous goods, minor damage to aircraft and/or equipment, minor injury to a person and/or damage to other property associated with the task Requires:

  • notification to management within 24 hours
  • institution of investigation procedures
  • recording in IRIS

 

May require (depending on severity of incident and regulatory requirements)

  • notification to relevant external agency (ATSB, NTSB, FAA, CASA, PHMSA, occupational health authority or any other identified authority). (see relevant regulation)
  • the obtaining of witness and participant contacts/statements
  • quarantining of relevant documents

 

1.1.6 Category 4 Occurrence

Hazard identified associated with the carriage of dangerous, operation of aircraft and/or equipment, personnel safety, and/or other property. Requires:

  • notification to management within 24 hours
  • institution of investigation procedures
  • recording in IRIS
